Today I have with me Dr. Dan Neff, one of our certified HODS clinics, who is doing an amazing job — not only with diagnostics but with diagnostics integrated into a very strong, clinical physical therapy practice. Thank you for being here, Dan.

Yeah, thank you. Thanks for having me.
Dr. Dimitrios Kostopoulos:
Let me ask you this first: Why diagnostics? Why did you integrate diagnostics into your practice?
Dr. Dan Neff:
So, for us, we were running into — you know, we’re a spinal specialist clinic — and we were running into these difficult cases of trying to figure out exactly where patients’ numbness and tingling or weakness were coming from.
We found we were relying heavily on either neurologists or other specialties to diagnose what was going on. And so, I got tired of that — of waiting and not really knowing what was happening.
I wanted the knowledge myself. I wanted the ability to take care of those patients completely. And so, for us, it became a way of completing the care. Right? I now can take a patient in with spinal dysfunction or an extremity issue and know that I can manage that patient to the best of my ability and get them the help they need.

So basically, what you’re saying is that diagnostics gave you the ability to be a true primary care provider for neuromusculoskeletal conditions. Would that be accurate?
Dr. Dan Neff:
Yes, yes. I would say absolutely. We don’t have a revolving door of patients. We’re a small rural practice, and we’re going to see the same patients over and over again. And so, I want to be able to fix those patients, not just treat their symptoms temporarily.

So how did you begin integrating diagnostics into your system? Because it’s not just about offering EMGs or ultrasound diagnostics — it’s about making sure the patient journey, the clinic flow, the decision-making, and the clinical pathways support it. Can you walk us through how that started?
Dr. Dan Neff:
We started with our leadership team — we have a group of five owners. We sat down and basically decided we were going to push diagnostics and truly get good at this.
So the first thing we did was re-educate our staff — including our therapists — on how to recognize when a patient needs testing. We developed protocols and clinical pathways. For example, if a patient comes in with radicular pain, weakness, or sensory deficits, that becomes a trigger.
We created screening forms that are part of the intake process now. Our front desk knows when a patient meets certain criteria, that they might be a candidate for EMG or MSK ultrasound.
So it’s a top-down effort. Everyone in the clinic is trained to recognize when diagnostics are appropriate.

That sounds like you were intentional — not just in clinical application but in culture and training. What was the effect on your clinical outcomes?
Dr. Dan Neff:
It drastically improved our outcomes. We had patients who were seeing us for six to eight weeks, and we weren’t getting the results we wanted.
Now, with the diagnostics in place, we can more quickly identify whether we’re dealing with a compressive neuropathy, radiculopathy, or something else entirely like a polyneuropathy. That changes how we treat the patient and when we refer them out — if we need to.
In the past, we’d exhaust their benefits before figuring out that PT wasn’t the answer. Now we know that by week 4 or 5 — and we adjust. The patient respects that.
Also, referring physicians started to respect us more. I’ve had neurosurgeons say, “We love your reports. They’re clear. They’re helpful. They’re actionable.” That wasn’t always the case before diagnostics.

We initially sat down as a leadership board and essentially said, the mission is to provide patients with a diagnosis — to figure out where their origin of pain comes from.

So, we created goals for in-house patient referrals. Essentially, that patient gets referred to us, they fill out a diagnostic screen, and during the initial examination, we assess physical therapy needs — spinal restrictions and everything else we evaluate daily.

When that patient has numbness, tingling, or weakness, we’ve found that outcomes completely change when we perform a diagnostic EMG. Because how many times do we see a neck pain patient with a double crush — maybe an elbow or median nerve issue across the wrist? That’s a double whammy. If we only focus on neck pain, we’re missing part of the outcome. The patient may still leave the clinic with numbness and tingling — and that’s not doing our job efficiently.

So, when the patient comes in, the DPT checks the paperwork: consent to treat, the No Surprises Act, any scripts. If the patient has numbness, tingling, weakness, or relevant medical history, we refer them for an EMG immediately.
Our therapists are trained to explain EMGs — how they work and why they’re beneficial. So during the patient’s first visit, both the EMG and their PT treatments are scheduled.

For in-house referrals, we give the patient a brief exam in person, but we send the EMG results back to the treating DPT, who then reviews them with the patient. The DPT doesn’t interpret results — but they walk through them so the patient continues to see them as their primary clinician.
That continuity builds long-term patient relationships. Patients know: “Every time I have pain, I’ll come back to Dr. Max or Dr. Chris Hall.” The diagnostic clinician (me or Adam) doesn’t take over that relationship — the PT remains central.

The treating therapists — even if they aren’t trained in electrodiagnostics — still communicate with the EMG therapist. That communication helps ensure the treatment plan reflects diagnostic findings.
Dr. Dan Neff:
Exactly. I had a therapist ask about borderline sensory testing, wondering if it indicated early polyneuropathy. That’s the goal — getting them to understand the report, the waveforms, the data, and what the EMG is actually saying. It elevates their clinical knowledge and improves practice quality.

Some PTs worry that referring to diagnostics will diminish the value of their evaluation skills. There’s a misconception that EMGs undermine their ability to identify functional pathology.
But it’s like when a dizzy patient walks in — you don’t just assume blood pressure is the cause. You check glucose, evaluate the vestibular system, and more. Similarly, EMG enhances your understanding — it doesn’t reduce your value.

Exactly. That’s why we have the DPT review the EMG with the patient — it reinforces their role.
We’re also realistic: not every patient will be healed through manual therapy. If we have a large disc pressing on a nerve, we won’t waste three months and the patient’s insurance benefits before referring for surgery. If they’re not better in 8–10 visits, we refer appropriately — and physicians respect that.
I once told an ortho group: “We sent you over 100 direct referrals last year, and you sent us none. We might start working with someone else.” It’s about mutual respect and outcomes.

Has offering diagnostics improved your clinic’s reputation?
Dr. Dan Neff:
Yes. I was once with a rheumatologist who introduced me to another as a PT. The guy barely acknowledged me. But then he added that I was board-certified in EMG — and suddenly the guy started asking EMG questions.
That’s the reality. Physicians often don’t understand demand and axon response. When you can explain that, your reputation goes up significantly.

We even use models — little spine demos — to show how joints move and how inflammation or fibrosis leads to nerve compression.
When segments don’t move, others compensate, causing overuse. We do spinal mobilization or manipulation to activate pain inhibition centers. We also check for pelvic imbalances, which can also contribute to symptoms.
Patients with peripheral neuropathy often benefit from spinal adjustments. By restoring mechanical function, you help improve neural input — and facilitate healing.

A study we published showed that in 61% of cases, EMG changed patient management. Do you see that too?
Dr. Dan Neff:
Yes. Without diagnostics, you might misinterpret polyneuropathy as just back pain. That completely changes how you treat. Same with C6 radiculopathy — active vs. stable processes affect whether you mobilize aggressively.
And in cases with superimposed carpal tunnel, you’d mobilize the hand, the fascia, the nerve — but only if you identify it.

I recently tested a physician who had failed carpal tunnel exams — but ended up having a severe median nerve issue. Clinical exams missed it. Without EMG, he wouldn’t have known.

How do you handle early-stage therapists who hesitate to use diagnostics due to concerns about increasing patient costs?
Dr. Dan Neff:
It’s cultural. We educate them: did you keep that patient for 40 visits and then send them for surgery? That’s not cost-effective.
With accurate diagnoses, care costs drop. We discharge earlier or make proper referrals.

Healthcare often operates in silos — physical therapy, imaging, surgery. But diagnostics helps you cut through all of that and get to the root cause faster.
I’d rather do 12 visits + EMG than 12 failed visits → MRI → surgery → repeat PT. That’s a failed system.

Yes! Do it early. Don’t wait until 12 visits fail. You’ll waste insurance-approved visits and delay patient recovery. Early testing gives us the best treatment plan.

We recently tested patients who had carpal tunnel surgery with no improvement — turned out they had full-length polyneuropathy. The surgery didn’t help because the nerve was already gone.
