Picture this:
You have two physical therapy clinics—Clinic A and Clinic B. They both treat the same Medicaid patient. One gets reimbursed at a loss, the other turns a profit. The difference? A financial loophole exploited by certain institutions, while private practice PTs are left holding the bag.

How the Provider Tax Loophole Works

Can you explain how this provider tax loophole actually works?

Dr. Larry Benz:
Absolutely. In short, a hospital pays a tax to the state, and the state uses that to access federal matching funds. The result? For every $1 a provider pays, they might receive $1.50–$1.70 back from the government. It’s essentially trading $1 for $1.70, completely legally.

This setup has existed in different forms for years. It’s sometimes referred to as a “money laundering scheme”, because while the state appears to contribute, the money actually originates from provider taxes.

The matching percentage varies by state, depending on population and Medicaid mix. Originally, Medicaid was meant to be state-funded with federal support, but it has flipped into one of the largest federal expenditures outside Medicare.

The system is called Intergovernmental Transfer (IGT) funding, and academic hospitals often benefit even more by getting outpatient reimbursements, too.

While IGT aims to bring Medicaid closer to commercial reimbursement rates, in some areas it exceeds them. That’s why nonprofit hospitals with academic ties are being paid more for Medicaid than commercial plans do—and certainly more than private practice clinics.

For private practice PTs, this means treating Medicaid patients at a loss, while hospital-based clinics profit.


Why Are Private Clinics Locked Out?

Why can’t private PT clinics access this reimbursement model?

Dr. Larry Benz:
Three key reasons:

  1. State laws don’t list PTs, OTs, or speech therapists as allowable Medicaid providers—because they didn’t exist when the laws were written.

  2. Provider taxes often apply only to physicians or hospital systems.

  3. Academic institutions get additional IGT perks for both inpatient and outpatient services.

No freestanding PT clinic qualifies for IGT unless they are hospital-affiliated and billing as such.


IGT vs. ACA and Privatized Medicaid

IGT mechanisms predate the Affordable Care Act, though the current form has been around for 5–7 years.
Some states use privatized Medicaid or capitated systems, where companies manage Medicaid patients. These private carriers aren’t subject to provider taxes and operate outside the IGT model.

Do you see an ethical issue in how reimbursement differs so drastically?

Dr. Larry Benz:
Yes—there’s a long-standing bifurcation in healthcare payment. For years, hospitals charged more for outpatient Medicare PT. That eventually leveled out, but private payers still reimburse hospitals 2–3x more than private clinics.

This impacts recruiting too—hospitals can offer higher salaries with lower productivity expectations, leaving private practices struggling to compete.


APTA, Advocacy, and Equitable Pay

What can be done to balance reimbursement across practice settings?

Dr. Larry Benz:
Advocacy is vital, but many issues are state-specific. We shouldn’t weaken hospitals—we should strengthen private practice reimbursement.

Private PTs offer better outcomes, have more specialists, and more residencies and fellowships. We bring greater value to the healthcare system and should be paid accordingly.


PT Profit Margins & Market Pressures

Margins in private PT clinics are slim—often 8–12%, sometimes less. A small Medicare cut can wipe out profits. Costs are up across the board: salaries, rent, supplies.

Post-COVID, therapist shortages and wage inflation hit hard. We’re delivering more value than ever, but payer rates lag behind economic reality.


Why Scale Matters

To survive, PT clinics need scale:

  • Shared services for billing, credentialing, and IT

  • Negotiated contracts with suppliers and payers

  • Recruiting and training infrastructure

That doesn’t mean selling to corporate chains—it means working together, building MSOs or IPAs, and maintaining independence with shared support.
