Steven, from your perspective, what is the primary value of RTM? Is it clinical outcomes, patient engagement, revenue—or all of the above?

It really has to be all three. For RTM to work long-term, all stakeholders need to see value: the patient, the clinician, the practice, and the payer.

From the patient side, we’ve seen improved adherence and completion of care. In a 480-patient analysis, patients using SaRa showed a 19% to 62% increase in plan-of-care completion. They don’t drop off after visit two because they’re sore or discouraged.

Clinically, providers walk into visits informed. Subjective data is already captured. Instead of asking, “How’s it been going?”, the conversation starts with, “I see your pain is improving, you’ve been consistent with your program, but stairs are still a challenge.”

From a practice standpoint, RTM generates high-margin revenue. Adding even $70 of net margin per episode can represent a 40–50% increase in bottom-line profitability.

What misconceptions do clinic owners have about RTM that keep them from getting started?

There are misconceptions—and there are real obstacles.

A big misconception is that RTM requires a massive time commitment. When implemented correctly, it should fit naturally into your workflow. Another misconception is that RTM must be home-exercise-based or app-based. That’s not true. The requirement is that you’re using software as a medical device to monitor the patient—not how that data is collected.

Some clinicians worry RTM increases audit risk. Is there evidence of that?

We’ve delivered millions in RTM reimbursement and have seen only one audit—and it was resolved with proper documentation. CMS clarified that in-person time discussing RTM data can count toward time-based codes, which actually reduces risk when used appropriately.

The most successful clinics separate enrollment from clinical care. Front desk staff or admin enroll patients. Clinicians use the data during visits. Some clinics also use a “mission control” role to manage first-line monitoring.

Yes. Enrollment can be done in just a couple of clicks through our Chrome extension or API integrations. Clinicians see RTM data directly inside the patient chart, and time spent reviewing data is logged automatically.

It’s not just Medicare. Many commercial payers—United, Aetna, Humana, Medicare Advantage, VA, TriWest—pay for RTM. Coverage varies by state for Blue plans, but adoption continues to expand.
