MSK Ultrasound Competency: Why Training Beats Guesswork
Buying an MSK ultrasound machine is easy. Developing the competency to use it correctly is what truly matters.
In this episode, Dr. Dimitrios Kostopoulos is joined by Dr. Mohini Rawat and Dr. Konstantine Rizopoulos to discuss why mentorship, structured training, report writing, and clinical integration are essential for becoming a competent musculoskeletal ultrasound provider.
Discover why true expertise goes far beyond weekend courses and YouTube videos—and how proper education protects patients, strengthens physician trust, enhances clinical decision-making, and builds a more successful practice.

[00:00] Buying an Ultrasound Machine Doesn’t Make You Competent
Would you allow an untrained surgeon to operate?
Of course not.
So why do some clinicians believe that buying a musculoskeletal ultrasound machine—or watching a few online videos—is enough to perform diagnostic imaging?
In today’s episode, we tackle one of the most important yet least discussed topics in diagnostic physical therapy:
Competency.
Musculoskeletal ultrasound is transforming healthcare, but only when performed by clinicians who have the knowledge, training, judgment, and mentorship to use it correctly.
Owning the equipment is easy.
Developing expertise is the real challenge.
[01:24] Why This Conversation Matters
Today’s discussion isn’t about scanning techniques or business growth.
Instead, it’s about something even more fundamental:
What does it actually take to become competent in musculoskeletal ultrasound?
Joining Dr. Dimitrios Kostopoulos are two internationally respected educators and co-founders of the American Academy of Musculoskeletal Ultrasound (AAMU):
- Dr. Mohini Rawat
- Dr. Konstantine Rizopoulos
Together, they discuss why competency—not technology—is the true differentiator.
[02:32] Buying the Tool Is Only the Beginning
Many clinicians purchase an ultrasound system after hearing about its clinical and financial benefits.
They become excited.
They begin advertising ultrasound services.
But one critical question remains:
Do you truly know how to use the tool?
Competency isn’t measured by ownership.
It’s measured by your ability to:
- Acquire quality images
- Interpret anatomy correctly
- Recognize pathology
- Produce accurate reports
- Communicate meaningful findings to referring providers
[03:53] Competency Is an Ethical Responsibility
Whether you’re a:
- Physical therapist
- Physician
- Orthopedic specialist
- Chiropractor
- Sports medicine provider
If you’re holding the ultrasound probe, you carry a professional responsibility.
Technical skill alone isn’t enough.
You also need:
- Clinical judgment
- Anatomical expertise
- Diagnostic reasoning
- Confidence grounded in education—not assumption
Patient safety depends on it.
[05:08] Dr. Mohini Rawat: Learning the Machine vs. Learning the Medicine
Dr. Rawat explains that learning ultrasound occurs in two very different stages.
First comes:
- Operating the equipment
- Probe handling
- Image acquisition
These skills can be learned relatively quickly.
The much harder part is understanding what you’re actually seeing.
That requires:
- Recognizing normal anatomy
- Identifying normal anatomical variations
- Distinguishing artifacts from pathology
- Knowing when something is truly abnormal
Without this foundation, clinicians risk both over-diagnosis and missed diagnoses.
[07:30] Dr. Konstantine Rizopoulos: The Biggest Mistake New Learners Make
Dr. Rizopoulos shares his own experience learning musculoskeletal ultrasound.
Like many clinicians, he attended an intensive course, returned home enthusiastic—and quickly realized he wasn’t prepared to use ultrasound confidently in practice.
The breakthrough came only after:
- Hundreds of practice scans
- Repeated study
- Continuous mentorship
- Ongoing case review
His experience highlights an important lesson:
Weekend courses introduce ultrasound. They do not create experts.
[10:30] The YouTube Education Problem
One of the most concerning trends discussed in the episode is clinicians relying almost entirely on:
- YouTube videos
- Short online tutorials
- Minimal hands-on instruction
Some begin scanning patients after only this limited exposure.
Many avoid writing formal reports or billing for their services—not because the technology lacks value, but because they lack confidence in the quality of their interpretations.
That uncertainty ultimately limits both patient care and professional growth.
[14:32] What Does Real Ultrasound Training Look Like?
According to Dr. Rawat, meaningful competency requires much more than classroom instruction.
Effective training includes:
- Structured mentorship
- Peer collaboration
- Progressive clinical education
- Ongoing feedback
- Regular image review
- Clinical case discussions
Most importantly, clinicians need repeated opportunities to compare their findings with experienced mentors.
That’s where true learning occurs.
[16:12] Why Fellowship Should Mean More Than Classroom Hours
The discussion then turns to fellowship education.
Not every program labeled a “fellowship” provides genuine fellowship training.
A true fellowship should include:
- Longitudinal mentorship
- Continuous faculty interaction
- Report review
- Clinical integration
- Performance assessment
- Progressive competency development
Education doesn’t end when the course finishes.
That’s where it begins.
[18:02] Building Clinical Competence Through Mentorship
The American Academy of Musculoskeletal Ultrasound Fellowship emphasizes:
- Foundational anatomy
- Progressive pathology recognition
- Clinical integration
- Standardized report writing
- Continuous mentor review
- Real-world application
Rather than overwhelming learners with information over a weekend, competency develops gradually through repetition, coaching, and refinement.
[19:21] Technology Can Support Competency—But Never Replace It
The conversation introduces SonoScript, a report-writing platform developed to streamline ultrasound documentation.
The software helps clinicians:
- Create structured reports
- Improve documentation efficiency
- Standardize reporting language
- Reduce administrative time
Importantly, technology assists the reporting process—but it does not replace diagnostic judgment.
Clinical reasoning still belongs to the clinician.
[25:20] Why Ongoing Case Review Matters
One of the defining elements of advanced training is continual case discussion.
Through regular webinars, report reviews, and peer collaboration, clinicians encounter:
- Challenging cases
- Unusual anatomy
- Rare pathology
- Clinical decision-making discussions
This continuous learning process transforms technical operators into confident diagnostic professionals.
[26:17] Why Reviewing Hundreds of Studies Is Essential
Early ultrasound education often focused on short introductory courses.
Follow-up surveys revealed a troubling pattern:
Many clinicians enjoyed the education but never integrated ultrasound into daily practice.
Why?
Because confidence never developed.
Today, mentorship programs require extensive review of real clinical cases before graduation, allowing students to:
- Learn from mistakes
- Refine technique
- Improve interpretation
- Develop consistency
Competence comes through repetition—not attendance certificates.
[28:14] Poor Competency Has Real Consequences
Misdiagnosis doesn’t simply affect one patient.
It affects:
- Patient trust
- Referring physician confidence
- Practice reputation
- Insurance relationships
- Professional credibility
Every ultrasound examination carries responsibility.
You’re not simply placing a probe on a patient.
You’re placing your professional reputation behind every report you sign.
[29:11] Are You Competent—or Simply Exposed?
Dr. Rizopoulos poses a powerful question:
“Are you competent—or are you simply exposed?”
Without proper preparation, clinicians often feel uncertain.
That uncertainty leads many to avoid:
- Billing
- Report writing
- Clinical decision-making
Confidence grows only when education and mentorship build genuine competence.
[31:06] Final Takeaway: Competency Is the Price of Admission
Musculoskeletal ultrasound offers enormous potential for improving patient care.
But success requires much more than owning sophisticated equipment.
It requires:
- Education
- Mentorship
- Assessment
- Accountability
- Clinical judgment
- Lifelong learning
Good scanning isn’t luck.
It is skill developed through disciplined training and experience.
Closing Message (32:08)
If you’re committed to integrating musculoskeletal ultrasound into your practice, don’t settle for simply learning how to operate the machine.
Become the clinician your patients—and referring providers—can trust.
Because in the end:
- It’s your name on the report.
- It’s your patient in the examination room.
- And it’s your professional reputation on the line.
Competency isn’t a bonus. It’s the price of admission.
