CMS has proposed a 2.4% increase in Medicare inpatient hospital payments for fiscal year 2027.
At first glance, that may sound like a routine payment update.
But buried within this proposal is something much more important:
CMS is revising and expanding the Comprehensive Care for Joint Replacement (CJR) model nationwide. The updated version is called CJRX.
If finalized, it would become the first nationwide mandatory episode-based payment model in fee-for-service Medicare.

Hospitals would be financially responsible not just for the surgery — but for the entire 90-day recovery period, including:
