Dr. Dimitrios Kostopoulos
Your patients love the results they get from your physical therapy treatments — but does the healthcare system value your work the same way? Today, we’re translating RVUs — relative value units — into plain English for physical therapists. We’ll show you how you can position your services for higher valuation through the RVU system, bridging the gap between clinical excellence and financial success.
In today’s episode of Your Practice, Your Money. See you in a moment.
00:01:20:23 – 00:02:21:18
Welcome to Your Practice Your Money, where clinical success meets financial freedom. I’m your host, Dr. Dimitrios Kostopoulos. If you find this episode helpful, please subscribe on Spotify, Apple Podcasts, or anywhere you listen — and share it with a colleague. That’s how we expand the profession’s knowledge.
Today’s topic: RVUs, the value-based future, and how physical therapists can win. We’ll define RVUs simply, highlight where PT is undervalued, and go through five specific moves to increase both reimbursement and clinical outcomes.
What Is an RVU?
00:02:21:20 – 00:03:12:16
RVUs are the scoring system payers use to price CPT codes. They’re built on three components:
-
Work RVUs
-
Practice expense
-
Malpractice cost
But here’s the hidden math:
Your payment = RVU × Conversion Factor × Geographic Adjustment
Meaning:
You can provide the exact same service in two different zip codes — and get paid differently.
Why Diagnostics Have Higher RVUs
00:03:12:18 – 00:04:38:08
Typical PT treatment codes have modest RVUs.
Diagnostic testing codes — EMG, NCS, MSK ultrasound — carry much higher RVUs because they require higher skill, equipment, time, and risk.
Many of these codes reimburse 3–10x more than a standard PT visit.
So here’s the big question:
If patients value what you do… does the system? And how do you speak the system’s language to get paid like it?
The Value Mismatch: Patients vs. the System
00:04:38:14 – 00:06:19:11
Patients value what we do:
-
Less pain
-
Restored function
-
Faster return to work or sports
-
Improved quality of life
But insurance companies value:
-
Code pricing
-
Documentation
-
MPPR edits
-
Visit caps
-
Authorizations
-
Outcomes-based metrics
While you deliver life-changing care, you often face flat per-unit rates.
This mismatch is exactly where diagnostic testing changes the math.
The RVU Pipeline: Why Some Codes Pay More
00:06:19:11 – 00:08:30:12
More RVUs = higher payment. Codes with greater complexity, equipment, and risk are valued more.
Contrast that with PT codes, which require:
-
Prior auths
-
Evaluations & re-evals
-
Visit limits
-
MPPR deductions
…and still yield modest reimbursement — the national average is ~$97 per visit (many clinics receive less).
Now compare that with:
-
EMG + NCS (45 minutes)
-
MSK ultrasound (15 minutes)
These diagnostic tests often require no authorization, involve one visit, and reimburse far higher due to their RVU structure.
This is one of the rare win–win scenarios:
Better care. Better payment.
Clinical Wins That Payers Understand
00:09:18:02 – 00:11:26:08
Diagnostics allow you to:
-
Triage quickly
-
Sharpen diagnosis
-
Reduce unnecessary imaging
-
Guide targeted loading
-
Improve precision & outcomes
Ultrasound visualizes soft tissue pathology in real time.
EMG/NCS clarifies radiculopathy vs. peripheral entrapment vs. polyneuropathy with high sensitivity and specificity.
And because payers recognize the clarity diagnostics bring, they reimburse accordingly.
Diagnostics also diversify your CPT mix toward codes with significantly higher RVUs and reimbursements, all within PT scope in 50 out of 52 states.
Five Moves to Improve RVUs & Reimbursement
00:12:03:12 – 00:15:30:15
1. Blend staff incentives — don’t reward only units
Tie 10–20% of compensation to outcomes, access, and patient experience to avoid paying bonuses on units insurers won’t reimburse.
2. Establish diagnostic testing in your practice
Higher-RVU services → higher revenue → more competitive salaries → improved staff retention.
3. Renegotiate with payers using outcomes + diagnostics data
Show diagnostic precision, improved pathways, reduced unnecessary imaging, and cost savings.
4. Create clear clinical protocols that trigger diagnostics
Medically, ~35% of PT patients qualify for EMG and ~70% for MSKUS.
Protocols ensure consistent utilization → higher blended RVU mix.
5. Recruit & retain clinicians who want to advance
Therapists trained in diagnostics create higher-value visits, stronger outcomes, and improve overall practice EBITDA.
Conclusion
00:15:30:17 – 00:16:32:07
Your patients already value you.
To get paid appropriately, you must speak the system’s language.
Use diagnostics to:
-
Improve diagnostic accuracy
-
Elevate care pathways
-
Strengthen your RVU mix
-
Demonstrate outcomes and cost-effectiveness
This leads to higher reimbursement, higher profitability, and a stronger EBITDA — positioning your practice for the future.
