I’ve been in this profession a long time. I graduated PT school in 1998 and started in clinics even earlier. Over the last 30 years, I’ve seen enormous change—but everything accelerated after COVID.
The healthcare system has changed. The public has changed. And the old way of running a PT practice is what’s holding many owners back financially.
At the same time, the opportunity has never been bigger. The wellness industry alone is projected to exceed $3 trillion in the next few years—and physical therapists should be leading that movement.

The public is investing in wellness because traditional healthcare feels broken. People are more informed, more proactive, and they’re searching online for solutions.
Fifteen or twenty years ago, patients relied almost entirely on physician referrals. Today, patients walk into healthcare saying, “Google says this,” or “ChatGPT told me that.”
If your practice isn’t visible online—if you’re not positioned as the expert—you lose before the conversation even starts.

The first step of financial planning is marketing. If you don’t consistently drive new patients into your business, everything else breaks—staffing, reimbursement, growth.
Most PT owners focus on operations and billing because that’s what they know. But without strong branding, positioning, and online visibility, you can’t raise your average revenue per visit, add cash services, or fire bad insurance contracts.

You can introduce cash-based services.
You can drop low-paying insurers.
And most importantly—you can pay your therapists better and recruit better talent.
Cutting marketing during slow periods is the exact opposite of what owners should do. Labor is your biggest expense. Marketing is your growth engine.
