00:00 – Welcome to Your Practice, Your Money

Dr. Dimitrios Kostopoulos

Welcome to Your Practice, Your Money, the podcast where clinical success meets financial freedom.

I’m your host, Dr. Dimitrios Kostopoulos, and in every episode, we bring you actionable insights to grow a thriving and profitable healthcare practice.

Are you charging what you are worth? Are you attracting and keeping the right patients? Could diagnostic testing be your next major revenue stream? And is your practice truly set up to work for you, not the other way around?

If you are a physical therapist, physician, chiropractor, podiatrist, or any type of healthcare entrepreneur, this show is your roadmap to next-level success.

So let’s dive in.

01:07 – Direct Access Was Only the Beginning

Dr. Dimitrios Kostopoulos

For many years, physical therapists have talked about direct access and autonomous practice.

Those of you who have been around for a while may remember APTA’s Vision 2020, which was all about practice autonomy. We achieved significant progress through direct access, but opening the door to autonomous practice and being prepared to walk through that door are two different things.

The opportunity we have right now with Primary Care Physical Therapy is about preparing physical therapists to responsibly function at the front end of the healthcare system—not simply as the person who treats the patient after someone else has already made the important decisions.

It is about developing the clinical judgment, diagnostic awareness, communication skills, and systems necessary to help determine what the patient actually needs next.

And this is exactly what I want to explore with you today.

03:03 – Primary Care PT Is a Level of Preparedness

Dr. Dimitrios Kostopoulos

Let’s start by eliminating one of the first misconceptions: that Primary Care Physical Therapy describes a particular practice setting.

Some people believe Primary Care PT is only for rural settings. It is not something that happens only in a hospital. It is definitely not reserved for rural areas where physicians may be unavailable, and it is certainly not some theoretical model we might see 20 years from now.

It is happening right now.

Primary Care PT is a level of preparedness.

A physical therapist in private practice in Manhattan, Florida, Kentucky, or anywhere else can begin functioning within a primary care model.

The question is whether that clinician has been prepared to take responsibility for the patient who walks through the door without somebody else first telling them exactly what is wrong.

And that distinction is extremely important.

Primary Care Physical Therapy does not mean that the physical therapist suddenly treats every medical condition. That would completely misunderstand the concept.

Primary care means that when a patient presents to you, you are prepared to determine the appropriate next step.

Sometimes the answer is to treat the patient.

Sometimes we need more information or diagnostic testing.

Sometimes the answer is to refer.

Sometimes the situation needs to be escalated immediately.

And sometimes our role is coordinating the patient’s care with another healthcare professional.

The real competency is not knowing how to treat everything.

The competency is knowing: What does this patient need next?

That is the heart of Primary Care PT.

05:46 – Direct Access Opens the Door, Primary Care Training Builds the House

Dr. Dimitrios Kostopoulos

Direct access was a tremendous advancement for physical therapy. It changed the legal relationship between the patient and the physical therapist.

But direct access by itself does not automatically create a primary care practitioner.

As I like to say:

Direct access opened the door. Primary care training builds the house.

Access without preparedness is incomplete.

If the patient can come directly to us, then we also inherit greater responsibility.

We must be better at screening. We must recognize when something doesn’t fit. We must understand when conservative musculoskeletal care is appropriate and when it is not.

And we must become increasingly comfortable making decisions in situations where there is uncertainty.

That is the next evolution of direct access:

Direct access, and then Primary Care PT.

07:09 – Does This Clinical Picture Make Sense?

Dr. Dimitrios Kostopoulos

This requires one of the most important changes in thinking.

The patient walking into your clinic is not a knee. They are not a shoulder. They are not a lumbar spine.

They are a person presenting with a clinical problem.

Back pain may be musculoskeletal, but not every presentation of back pain is simply a back problem.

Dizziness may be vestibular, but not every patient with dizziness belongs in a vestibular rehabilitation program.

Weakness may represent deconditioning, but it could also indicate a neurologic process or another medical issue.

Primary Care PT begins with a deceptively simple question:

Does this clinical picture make sense?

Does the history fit?

Does the examination fit?

Does the patient’s symptom presentation fit?

And when those things do not fit, are we prepared to recognize it?

That’s where clinical maturity begins.

Traditionally, many physical therapists approach the patient by asking, “Can I treat this?”

But the more important question is:

“What does this patient need right now?”

Those questions sound similar, but they represent very different levels of responsibility.

“Can I treat this?” is about us.

“What does this patient need now?” is about the patient.

When we function as first-contact practitioners, our responsibility begins with screening, then deciding, referring when necessary, and coordinating care.

Treatment may absolutely be the correct answer, but treatment should be the consequence of good clinical decision-making—not simply the default response because the patient happened to walk into a physical therapy clinic.

09:44 – Diagnostics: The Goal Is Better Decisions

Dr. Dimitrios Kostopoulos

Now we come to an important part of the model: diagnostics.

There are many situations where history and physical examination give us a strong clinical impression, but we still need additional information to make the right decision.

That is where diagnostic tools can dramatically strengthen patient management.

I’m talking about:

  • Electrodiagnostic testing
  • Nerve conduction studies
  • Musculoskeletal ultrasound
  • Appropriate medical screening

The objective is not to perform more tests. I want to emphasize that.

The goal is not more testing. The goal is better decisions.

Diagnostics should clarify uncertainty. They should help distinguish between competing possibilities, influence patient management, and help us determine the most appropriate pathway for that patient.

The difference between a guess and a decision often lies in better data.

There is a simple way to think about how a physical therapy clinic can function within the primary care model:

Triage. Screen. Clarify. Guide.

First, triage. Can the patient access the practice quickly, potentially with a same-day appointment when they develop a musculoskeletal problem?

Then, screen. Does the clinical presentation make sense? Are there red flags? Do we recognize the pattern?

Then, clarify. If we don’t have enough information, what is the appropriate next step? Do we need EMG or nerve conduction studies? Musculoskeletal ultrasound or imaging? Laboratory testing through another provider? A physician consultation?

Finally, guide. Once we understand what is happening, we help the patient reach the right care at the right time.

12:37 – Clinical Value and Economic Value

Dr. Dimitrios Kostopoulos

It is also important to discuss the economics of the practice and something many healthcare professionals are sometimes uncomfortable discussing: the relationship between clinical value and economic value.

There is nothing wrong with a healthcare organization or healthcare professional becoming financially stronger because they provide greater value.

Patients want clarity. They want somebody who can tell them what may be happening and what they should do next.

Physicians want confidence in the professionals to whom they refer.

As practice owners, we need sustainability.

And physical therapists want meaningful careers with opportunities for advancement.

When a practice develops higher clinical capabilities, stronger diagnostic integration, and greater professional authority, those capabilities can create economic value for the system.

Financially healthy practices can invest in people, training, technology, growth, and better patient care.

14:13 – Differentiation Is Economic Protection

Dr. Dimitrios Kostopoulos

Here’s a challenge every physical therapy practice owner should think about.

Look at the websites of 10 different physical therapy practices.

What do most of them say?

“We provide quality care.”

“We provide hands-on treatment.”

“We have state-of-the-art equipment.”

“We provide personalized attention.”

None of those things are bad.

The problem is that everybody says them.

If the market cannot understand why your practice is different, the market will naturally assume you are basically the same.

When the marketplace sees all providers as interchangeable, the discussion eventually becomes about convenience, price, insurance participation, or location.

And that is not where we want our profession competing.

True differentiation comes from capability.

Primary Care Physical Therapy gives a practice the opportunity to say:

“We do something meaningfully different here.”

Differentiation is not decoration.

It becomes economic protection.

For years, the message of physical therapy has essentially been, “We treat pain and function.”

And of course we do. But millions of other healthcare professionals and healthcare businesses also say they treat similar things.

Primary Care PT allows us to communicate something substantially different:

“We help determine the right next step.”

Think about the authority contained in that statement.

A patient doesn’t necessarily know whether they need physical therapy, orthopedics, neurology, imaging, or something else.

Instead of asking patients to figure out the healthcare system first and then come to us, we’re saying:

“Come to us. We are Primary Care PT, and we will help you determine the appropriate healthcare pathway.”

That represents a very different relationship with the patient and a different level of professional authority for the physical therapist.

17:00 – Preparing Physical Therapists for Greater Responsibility

Dr. Dimitrios Kostopoulos

That brings us to the question of training.

If we expect physical therapists to assume greater responsibility, we also have to provide them with a legitimate pathway for developing those competencies.

And that is why we created the Primary Care Physical Therapy Residency.

The purpose of the residency is not simply to add another credential after somebody’s name.

The purpose is to systematically prepare physical therapists for first-contact decision-making.

The program is designed for the therapist who says:

“I don’t just want greater autonomy. I want to be prepared for the responsibility that comes with it.”

Our first cohort is starting in September, and you can learn more at PrimaryCarePT.com.

I also want to bring in Dr. Korry Wheeler, director of the Primary Care Physical Therapy Residency program at Hands-On Diagnostics.

18:19 – Is the Patient in the Right Place?

Dr. Korry Wheeler

You mentioned that Primary Care Physical Therapy is about more than simply asking, “Can you treat this patient who is coming into the clinic?”

When I first started looking into what Primary Care Physical Therapy really is, I had the same thought. I went to PT school. I’m confident in what I do. I can treat anybody who comes into my clinic.

And I think that’s true for all of us.

But the real challenge is: Is that the most appropriate place for that patient to be?

Can we recognize things that are truly not musculoskeletal but may be mimicking a musculoskeletal condition?

Considering the patient’s comorbidities, is this the right place for them? Do they need to be somewhere else? Do they need to be monitored more closely?

Those are areas where all of us can have either a little work or a lot of work to do.

That’s what the primary care program is designed to address.

It doesn’t really focus on how to treat patients. It focuses on whether they are in the right place—and if they’re not, where we need to send them.

We focus much more on:

  • Lab values
  • Vital signs
  • Physiology
  • Pharmacology
  • Diagnostic imaging
  • Diagnostic ultrasound
  • EMG
  • Nerve conduction studies

These tools help us make the decision: Is this patient in the right place, or do I need to get them to the right place?

19:54 – See More, Ask Better, Decide Stronger

Dr. Dimitrios Kostopoulos

Thank you, Korry. That’s awesome.

What we are trying to build through this residency education can be summarized in three things:

See more. Ask better. Decide stronger.

See more means developing the ability to recognize what might otherwise be missed.

Ask better means improving the history, screening process, clinical reasoning, and differential diagnosis.

And decide stronger means being able to take the information in front of you and determine an appropriate course of action with greater confidence.

That requires competency in:

  • Medical screening
  • Differential diagnosis
  • Referral judgment
  • Care coordination
  • Professional communication

Becoming a primary care practitioner is not about accumulating facts.

It is about becoming better at making decisions.

21:06 – Compete on Opportunity, Not Salary Alone

Dr. Dimitrios Kostopoulos

This creates another significant opportunity.

You can begin positioning your physical therapy practice as a genuine front door within your community.

Your organization can become an affiliated training environment. You can create a stronger talent pipeline, integrate diagnostics more effectively, strengthen relationships with referral partners, and differentiate your practice.

You can also offer ambitious physical therapists a pathway toward advanced professional development and board certification.

That last point is extremely important for recruitment and retention.

Most practices are trying to recruit physical therapists by competing on salary, but salary alone is a very difficult competition to win, especially against hospitals.

What happens when you can tell a young therapist:

“Come here, and we will help build your career”?

Now you are competing on opportunity.

And that is a much more powerful value proposition.

22:30 – Expanded Capability Must Be Paired With Standards

Dr. Dimitrios Kostopoulos

What we’re talking about is not simply an academic discussion about what physical therapy might become someday.

The paradigm is already moving. It’s here.

Within HODS—Hands-On Diagnostics—we are seeing affiliated practices across more than 30 states.

We have seen how diagnostic testing and information can change patient management. We have seen very strong patient satisfaction, and we have seen how diagnostic capabilities can create an entirely different economic model for the visit.

But the message is not simply about the numbers.

Expanded capability must always be paired with standards.

You can use the numbers as proof, and you can use standards as protection.

If we are going to expand the role of physical therapy, we must simultaneously expand education, competency, quality assurance, and accountability.

Growth without standards is dangerous.

Growth supported by standards is professional advancement.

23:46 – Primary Care PT and the Value Optimization System

Dr. Dimitrios Kostopoulos

I want to make an important distinction.

Primary Care PT is the clinical role, but clinical excellence alone does not automatically create a successful practice.

A practice also requires appropriate systems.

That is where our Value Optimization System, VOS, comes in.

VOS looks at the organization through five interconnected areas:

  • Clinical education
  • Clinical advancement
  • Practice expansion
  • Organizational stability
  • On-site diagnostic services

Primary Care Physical Therapy strengthens the clinical identity of the organization, while VOS provides a framework for transforming that clinical value into a sustainable practice model.

The objective is to build a practice capable of creating value, communicating value, and sustaining value.

So the question every practice owner should ask is:

“Can my team safely receive the patient first?”

Not, “Could they probably treat the patient?”

Not, “Are we good therapists?”

But if a patient walks through the door before seeing anybody else in the healthcare system, can my team safely evaluate that patient and determine the appropriate next step?

If the answer is yes, that’s excellent. Keep developing that capability.

If the answer is “not yet,” there is nothing wrong with that. It simply identifies your next strategic opportunity.

Clinical advancement leads to practice differentiation.

Practice differentiation creates professional authority.

And professional authority changes the position of physical therapy within the healthcare marketplace.

25:53 – The Future Requires Clarity, Leadership, and Authority

Dr. Dimitrios Kostopoulos

I believe the future will increasingly reward healthcare professionals who can operate closer to the beginning of the patient’s journey.

For too long, physical therapy has often been positioned downstream.

The patient sees somebody else. A decision is already made. An order is written. Then the patient arrives at physical therapy.

There will always be a role for referral-based physical therapy, of course.

But our profession is capable of much more.

The future requires three things:

Clarity. Leadership. Authority.

Physical therapy has always been a profession of movement. We help people move better, restore movement, and preserve movement.

But now we also have the opportunity to become a profession of clinical direction—helping patients navigate what should happen next.

That is a significant evolution in our professional identity.

Build the front door before the market demands it.

28:54 – Residency Clinical Hours and Program Structure

Dr. Dimitrios Kostopoulos

A question we receive is: Are clinical hours required for the program, or is it mostly coursework?

There are clinical-hour requirements, but those are completed in your own office as you see your own patients.

Those patient-care hours count toward the clinical-hour requirement.

Even someone working part-time can meet the requirement. You do not have to work full-time to complete the clinical hours.

ABPTRFE has approved the residency program for candidacy, and our first cohort is starting in September.

Dr. Korry Wheeler

The program includes approximately 400 educational hours, including three required weekends in either Astoria or Clearwater, depending on the track you select, as well as online coursework.

There are also 1,550 patient-care clinical hours.

As Dr. Kostopoulos mentioned, those clinical hours are completed in your own clinic with patients you are already seeing, specifically with experiences geared toward primary care.

So the time away from your clinic is very small. Most of the residency is incorporated into what you are already doing every day.

There is also guided learning around the patients you are seeing—for example, understanding how medications may be affecting them, incorporating vital signs and lab values, and determining whether they remain appropriate for physical therapy or need to be referred elsewhere.

Much of the mentoring can be completed asynchronously through communication with your mentor online, while in-person mentoring takes place in your own clinic.

31:36 – Can Any Clinic Support Primary Care Training?

Dr. Korry Wheeler

One question was, “What if my clinic doesn’t allow me to practice primary care?”

There really shouldn’t be a clinic that prevents you from applying primary care thinking.

You may not necessarily have direct access without referrals, but with every patient you evaluate, you are determining whether that patient should indeed be there or whether they need to be referred elsewhere.

Whether it is officially called primary care or not, those experiences can count when you are in a position to make those decisions.

32:12 – Do Residents Need to Perform EMG?

Dr. Dimitrios Kostopoulos

Another question is: “What if my clinic doesn’t allow me to perform EMG?”

You do not need to perform EMG in this program.

You are not going to be trained to perform EMGs as part of the residency. You will have some exposure, but performing EMG is not a program requirement.

Dr. Korry Wheeler

We’re going to teach you enough to understand an EMG report so that you can interpret it and use that information in your clinical decision-making.

There will be significant exposure and training in ultrasound.

For EMG, you’ll attend one of our introductory courses to gain enough exposure to understand the testing and how it relates to patient management.

32:56 – Closing Remarks
