From early on, my co-founder Ravi Atreya and I were idealistic—we got into healthcare because we wanted to help patients. And we’re both nerds—we were deeply into computer science. Vanderbilt had built its own EHR system, and we saw so much potential for how tech could help clinicians.
But in real-world practice, we saw a massive gap between that vision and what was actually happening. Documentation was a burden. Systems weren’t smart. We wanted to fix that—build smarter tools that actually helped clinicians take better care of patients.

We both handled technical design—he built the back-end, I focused on the front-end. A big lesson we learned: healthcare data is incredibly messy. Even at top institutions, just organizing and cleaning the data for machine learning took 80% of the work. We realized if it takes that much effort to solve one use case, scaling to all clinical scenarios would be impossible—unless we rethought the foundation.
So we built that foundation—starting with clean, organized, structured data. That led to models for compliance, CPT coding, and eventually the real-time documentation assistant.

Pedro: It’s as simple as clicking a button. You’re in the EMR like normal, hit “record,” and see your patient. The system listens. You talk normally—not like dictation. After the session, click “stop,” and 6–10 seconds later, a full draft note is ready.
The quality jumps because the system hears everything. It captures skilled care language, assessments, patient education, and objective findings—because you verbalize it naturally during the visit. Patients also love it because they understand more about their condition as you talk.

Pedro: Our documentation assistant is called Sidekick. But we actually started with compliance. We built custom models to evaluate compliance against Medicare and industry standards. We also run large-scale evaluations against millions of notes to train the model on functional, defensible goals and language. It’s built right into the drafts your therapists receive.

Pedro: CPT coding is tricky—especially for therapeutic activities or neuro codes. We trained our model on vast datasets to map documented justification to the correct CPT codes. If the note justifies a higher-value code like neuro re-ed, the model flags that. If the documentation doesn’t match the code used, it flags that too.
And this isn’t about upcoding—it’s about accuracy. We’ve seen a 3-to-1 ratio where therapists default to TherEx when neuro or therapeutic activity would have been appropriate—and reimbursed higher.
